What is Baraclude/Entecavir?

Baraclude (generic Entecavir) is a antiviral medication that is commonly used for the treatment of Hepatitis B specially with patients who have liver damage. This medication is not limited to treating hepatitis B, but it is also used for the treatment of HIV/AIDS and HBV infection.

How does Entecavir works?

Baraclude is a reverse transcript inhibitor whose main function is to prevent the hepatitis B virus from multiplying, thus reducing the hepatitis B virus in the body. Although entecavir reduces the hepatitis B virus, but it is not a cure for the disease. Entecavir doesn't prevent liver damage or reduces the risk of developing liver cancer.

Efficacy of Entecavir

According to a study, Baraclude (generic Entecavir) had an unexpected lower incidence of hepatocellular carcinoma in the patients suffering from hepatitis B. Also, it was observed that treatment with Entecavir led to 64% lower incidence of HCC among the patients who were suffering from chronic hepatitis B without cirrhosis.

In order to evaluate the impact of Entecavir (Brand Baraclude) with antiviral therapy on HCC Development, the study used the Risk Estimation of HCC in chronic Hepatitis B model. This model provides virtual control group of patients whose HCC incidence is calculated based on the clinical variable's availability. 841 patients were enrolled in this study; But at the end data from total of 646 patients were analysed. Among these 646 patients, 420 were male, 542 were Asians and 232 were positive for hepatitis B e antigen.

During the study, 17 patients were diagnosed with HCC- of which 8 patients were suffering from cirrhosis and 9 patients were not suffering from cirrhosis. During the first year of the study, 3 patients developed HCC without cirrhosis, whereas the REACH-B model in the first year estimated 4.7 cases. This led to a standardised ratio of 0.64. But this ratio had fallen to 0.37 during the 4 years of the study.

HCC incidence was lower than predicted, when the results were compared between patients with and without cirrhosis.

REACH-B model predicted 30.2 cases of HCC incidence during 8.2 years of follow up. But the actual figures were almost half 17 cases. This showed HCC incidence was lower than predicted.

In the end it was observed by the researchers that Entecavir antiviral therapy is associated with lower cases of HCC incidence. Although, the risk of HCC still persisted in the chronic hepatitis B patients. Vigilant surveillance remain warranted in HCC patients regardless of their reaction/response to Entecavir antiviral treatment.

Who should not take Entecavir treatment?

• Patients who are allergic to Entecavir (Brand Baraclude) are advised not to take this medication.
• Also it is very important to clarity on your HIV status, because this medication can significantly complicate the treatment for HIV.
• If you have an untreated or unrecognised HIV infection, do not start this medication without consulting an infectious disease specialist.
This medication comes in 2 forms – Oral tablets and Oral solution i.e. Entecavir 0.5 mg and Baraclude 1 mg and Baraclude 0.5 mg / ml.
